How to Prepare an Application for Transit Benefit

Form ITA-2064, “DOC Application for Transit Benefit”, along with the Mass Transit Expense Work Sheet, is available on the ITA Intranet “Our Place” at https://ourplace.ita.doc.gov/ using Adobe Acrobat software, must be completed to apply for transit benefit.

 

  1.  When you access ITA-2064, click on “Headquarters” or “Domestic Field."

 

  2.  Complete name, last four digits of social security number, grade/rank, home address, work address, office, room number, mail stop and telephone

       number.

 

  3.  Check applicable boxes for “Current Mode of Transportation for Commuting”.  

 

  4.  Check applicable boxes for “Mass Transit Benefit Mode of Commuting”.

 

  5.  Check applicable box for “Do You Receive Reduced Fare Public Transportation Rates”.

 

  6.  Complete and sign the Mass Transit Expense Worksheet.

 

  7.  Sign and date application.

 

  8.  Type accounting classification code.

 

  9.  Enter appropriate dollar value of fare media requested.

 

10.  Obtain supervisor’s signature.

 

11.  Obtain Program Resource Coordinator’s signature.

 

12.  Submit the completed form to Sharon Russell, Transit Benefit Coordinator, Room 4001.  For the domestic field, fax form to Debra James at

      937-226-8221.
