to Change Information on Your Approving Official Purchase
Card Account

To change
information on the Approving Official Account form ITA 2063,
"Government Approving Official SetUp/Maintenance", available using the
Adobe Acrobat E-forms Website via "Our Place" at http://ourplace.ita.doc.gov.
1. SECTION
I. Type of request:
Indicate an 'X'
by the action you are requesting .
2. SECTION
II. Approving Official Information
- To change
information for an approving official, type in only
the information to be changed. Leave all other
information fields blank.
- To Cancel an
approving official, type 'TURN OFF REPORTS' after the name.
3. SECTION
III. Reporting Parameters
- Indicate account number - cardholder's card number.
- To move an
approving official to another hierarchy code, enter one
of these codes on the 3rd blank line for the Reporting
Hierarchy. Also indicates levels 5 & 6 of the hierarchy levels.

- To move or
cancel an approving official enter the last 5-digits of
the approving official account number on the 4th blank line, the levels
5 & 6 on the following 2 lines.
4. SECTION
IV. Approving Official Signature
- Type the
approving official's name.
- Obtain the
signature of the Approving Official and indicate the
date.
- Upon submission, the
Approving Agency Program Coordinator will sign.
5. Signature
DUS, CFO&DOA, AS or DAS
- Office of the
Under Secretary, obtain the signature of the DUS.
- Program
Units, obtain the signature of the approving official's
supervisor at the DAS-level or higher (DAS/AS).
- Administration
Offices, obtain the signature of the CFO and Director of
Administration.
6. Headquarters
and Overseas submit completed forms to Robin Carpenter, ITA
Purchase Card Coordinator, in Room 4001, and domestic field
offices submit forms to the servicing ASCs.
