To Change Information In Your Purchase Card Account

To change information on Purchase Card
accounts, use form ITA 2061, "Government Purchase or Travel Card Maintenance" , available using the Adobe Acrobat E-forms
Website via "Our Place" at http://ourplace.ita.doc.gov.
1. SECTION I.
- Identify the account to be updated.
- Type the cardholder number.
- Type the name of the cardholder as
it appears on the card.
2. SECTION II.
- Type in only the
information to be changed. Leave all other information fields
blank.
- Put an "X" by Cardholder Information
Change (Section III). Complete in Section III only the information to be
changed in the cardholder account. Leave the other information fields
blank.
- Put an "X" by Hierarchy Change
(Section IV ) to move a cardholder from one hierarchy code to another
hierarchy code. Enter one of these codes on the 3rd blank line for the Current
Reporting Hierarchy and the New Reporting Hierarchy:

- To move a cardholder from one
approving official to another approving official, enter the last 5-digits
of the current approving official's account number on the 4th blank line for
Current Reporting Hierarchy. Enter the last 5-digits of the new
approving official's account number for New Reporting
Hierarchy.
- Put an "X" by Dollars per Cycle
Limit Change (Section V) to change the monthly dollar limit (not to
exceed $10,000).
- Put an "X" by Account Closure
(Section VI) to cancel the card account.
3. Approving Official
Signature
- Obtain the signature of the
Approving Official.
- Obtain the signature of the
Approving Official's supervisor at the DAS-level or higher (DUS, CFO&DOA,
A/S or DAS).
4. Headquarters and overseas staff
submit completed forms to Robin Carpenter in room 4001, and domestic field
offices submit forms to the servicing ASC.
