How to Store and Retrieve Items at the Commerce Warehouse

Requests to store or retrieve items at the Department of Commerce's NOAA Warehouse Services should be submitted on a Form CD-540, "Warehouse Issue/Pickup Request."
1. Complete the Form CD-540 "Warehouse Issue/Pick-up Request (click for sample)," available on Ourplace.
SIGNATURE OF APPROVING OFFICIAL: Include telephone and Room number.
SPECIAL INSTRUCTIONS: Provide name and telephone number of personnel authorized to store or retrieve items.
ITEM NUMBER: Each item must be assigned an item number. The item number can be up to 15 characters in length (e.g. ITAIAFTZ-1, ITAIAFTZ-2...).
TITLE OR DESCRIPTION OF ITEM(S): Enter a brief description of the item being stored or retrieved (e.g.. Foreign Trade Zones Applications).
2. Fax the completed CD-540 form for approval to the Office of Organization and Management Support (OOMS) at 202-482-1584 or deliver it to Room 4001. OOMS will then forward the CD-540 to the Warehouse. Orders should be picked-up or delivered within two working days of approval to the requester's address listed on the CD-540.
3. Sign and date the CD-540 when the order is received from the Warehouse.
4. Inquiries or problems with warehouse orders should be directed to Larry Linkoff at 301-372-2923. The fax number for the Warehouse is 301-372-2912.
5. Questions concerning storage/retrieval procedures and emergency requirements should be directed to the Office of Organization and Management Support.
