How To Prepare A Citibank Travel Card Application

The General Services Administration (GSA) has a contract with Citibank to issue travel charge cards to eligible employees of the Federal Government. With GSA requiring mandatory use of the Citibank travel card by all Department employees who travel, it is important that travelers apply for and use the card. This card will be issued to you without an annual fee and without a personal credit check. The card will afford you financial flexibility and convenience while traveling on official Government business.  ITA staff who travel two or more times a year must have a Citibank Travel Card. The Citibank Travel Card may only be used to obtain a cash advance, purchase airline tickets, hotels, restaurants, automobile rental and other expenses incident to official travel. The card may be used only for official government travel. Personal use of the card is strictly forbidden.  

To obtain the Citibank Travel Government Charge card ITA employees: 

1.   Must take the mandatory on-line travel card training at http://www.gsa.gov/Portal/gsa/ep/contentView.do?P=FCXM&contentId=13033&contentType=GSA_BASIC

2.  Complete the Citibank Government Travel Card, ITA 2065, application.

Section II - Completed by Agency/Organization Coordinator

Section III - Completed by Agency/Organization Coordinator

Section IV - Cardholder Information

  • Type Full name  – First, Middle Initial and Last.
  • Type Name of Agency.  
  • Type home phone, business phone and business fax number.
  • 4th Line Embossing: - DO NOT COMPLETE
  • Type Your home mailing street address.  PO Boxes are not allowed.  
  • Type Social Security Number. 
  • Type business address.     
  • Type Mother’s maiden name.   
  • Type business e-mail address.
  • Type Date of Birth
  • Section V - Sign and date application

    Section VI - Completed by Agency/Organization Coordinator

    Section VII - Completed by Agency/Organization Coordinator

    3.  Submit the application along with the signed employee acknowledgement form and certificate of training to:
