How To Dispose of Personal Property

Surplus personal property is furniture, equipment, materials, and supplies no longer required by your office. Complete form CD-50 available on your network using Formflow filler software to dispose of Personal Property. The CD-50 must be used to surplus all personal property including property that is not barcoded.

1. Enter the date and under 'Purpose: "click on 'Excess".

Enter the information applicable to the Bureau RELEASING the property on the side of the form marked FROM:

2. Bureau: Enter ITA/Unit/DAS/Office.
3. Organization Code: Enter your 18-digit personal property code beginning with 55.
4. Location: Type 110010001 for headquarters.
5. Custodian name and Cust #: Enter the property custodian's name and number.
6. Address: Enter the property custodian or contact's address, phone number and room number where the property is located.
7. Accounting Class: Enter the releasing office's appropriation code.
8. Enter the Barcode Number, Manufacturer Name, Description, Model Number, Serial Number and Original Acquisition Amount. (If the item is not barcoded enter the description of the item and the original acquisition amount.)
9. Enter the Condition Code in the column marked with an asterisk *. Use one of these condition codes: 1 Excellent, 4 Usable, 7 Repairable, X Salvage, or S Scrap. These new condition codes replace the condition codes on form CD-50.

Enter the information applicable to the Bureau RECEIVING the property on the side of the form marked TO:

10. Bureau: Enter NOAA Warehouse. Leave the remaining information blank.

The office property custodian should:

11. Obtain the signature of the releasing property custodian and retain a copy of the CD-50 in your property records.
12. Send the CD-50 to Room B-511, in an inter-office envelope, addressed to HCHB Shipping and Receiving.
13. Send a copy of the signed CD-50 by regular mail:

14. Delete the property record(s) in the DOI Flash Point Fixed Asset System (FAS).
15. Call the HCHB Shipping and Receiving Section on ext. 6215 if the property has not been removed in 48 hours. Do not place Personal Property in the hall.
