How To Obtain A $3,000
Purchase Card

The Purchase Card allows
authorized staff to order and pay for small purchases that are $3,000 or less
for a single item. Personal use of the card is strictly
forbidden. To apply for a Purchase Card, complete the ITA 2058 -
Government Purchase Card Set-up Form by logging into "Our Place" at https://ourplace.ita.doc.gov/ and
proceed to the E-forms Website. In addition to filling out the form, new
applicants must take the GSA SmartPay Purchase Card training at http://apps.fss.gsa.gov/webtraining. Click on GSA SmartPay Purchase
Card and the Section 508 mandated training at http://www.section508.gov/. An
example of the ITA-2058 is attached.

HEADQUARTERS submit completed ITA-2058 form and
a copy of your GSA SmartPay and Section 508 certificates to Robin Carpenter, ITA
Agency Program Coordinator, in Room 4001. DOMESTIC FIELD offices
submit completed ITA-2058 form and a copy of your GSA SmartPay and Section 508
certificates to your servicing ASC. OVERSEAS FIELD offices submit
completed ITA-2058 form and a copy of your GSA SmartPay and Section 508
certificates to your servicing Regional Director’s office.
