- How to Submit Personnel Actions Using
Standard PDs

The DOC PD Library Standard PD
program prompts a number of Personnel Action process and documentation changes,
including use of a new "Performance Appraisal Record," Form ITA-2050, and
"Classification and Performance Management Record, Form CD-516 (front page
only).
To submit a Personnel Action under
the Standard PD program:
- Complete an SF-52, "Request for
Personnel Action”.
- Complete the front page only of
the CD-516 "Position Classification and Performance Management Record".
- Select the Standard PD for the
grade, series and title of position involved.
- Enter the organizational location
of the position in Section I, Introduction.
- Enter any special working
conditions (excessive travel, shift work, hazards, etc.) and/or mandatory
specialized abilities or knowledge requirements in Section IV, Unique Position
Requirements.
- Include the Performance Appraisal
Record for the position, assuring that the plan shows Critical Results and
Supplemental Performance Indicators consistent with the grade and title of the
Standard PD being used. (For multi-level recruitment submit standard PDs for
all levels, but Performance Appraisal Record only for the highest advertised
grade level).
ITA OHRM will return the original
performance appraisal record. Upon
receipt, supervisors are to: (1) discuss Performance Plans with employee and (2)
provide the employee with copies of both the Performance Appraisal Record and
PD.
