The Office of Organization and Management Support is responsible for the administration of a uniform ITA Forms and Stationery Management Program.  The ITA Forms and Stationery Management Officer maintains central control of the issuance of new, revised or reprinted  forms.  Questions regarding ITA forms and stationery should be directed to 202-482-3585.   This catalog identifies all current ITA forms and stationery and serves as a reference guide for ITA employees.  All revisions to this document should be submitted to the OOMS.   Only forms listed in this catalog are current and official. 

ITA envelopes and stationery are stocked and distributed by the Department of Commerce's NOAA Warehouse Services in Waldorf, Maryland and should be requested on a CD-540 "Warehouse Issue/Pick-up Request" form (See How to Order ITA Forms, Envelopes & Stationery).   ITA forms (for internal use) and Public Use Forms with a single asterisk are available via Our Place  website and the Public E-form website, respectively.  All other forms listed in this catalog are in hardcopy and can be obtained from the designated office listed next to the form.

Reprinting of  ITA Stationery:

Submit Form SF-1, "Printing and Binding Requisition" and a template of the stationery that is to be printed to the ITA Forms and Stationery Management Officer in Room 4001 for clearance before a requisition number is issued.  

Requests for New or Revised Form:

            For ITA Forms (internal use) 

Submit a draft copy (in electronic format) of the new or revised form to the ITA Forms Management Officer (Room 4001) for electronic .pdf design.   

            For Public Forms

Request for Office of Management and Budget (OMB) Approval of Information Collection Requests placed on the Public:  Submit a completed Form OMB 83-1, "Paperwork Reduction Act Submission," including supporting material to the ITA Reports Clearance Officer (Room 4001).  (Refer to ITA Administrative Instruction 6-11, Clearance of Collections of Information from the Public)
