Training Request Form (SF-182) and Procedures

SF-182 Registration forms should be submitted to HCHB Room 2006. You may fax your approved form to 202-501-6159. Registration for courses are on a first come, first serve basis unless otherwise indicated.

What pages of the SF-182 are required to be submitted?

Please submit only completed, signed and approved pages 1 and 2 of the SF-182, and if applicable, pages 4 and 5.

Social Security Number (SSN):

Only the last 4 digits of the social security number (SSN) in Section A, box# 2 are required. For example: XXX-XX-5432

Date of Birth (DOB):

Please note that it is not required to include the DOB.
